Do the bolts come with it, and if not what size bolts (thread. width and length) are required? Also, does this require a key and is that provided?

on after market body is there problems with clutch arm pedal not clearing body support and not being able to slide on frame support shaft completely or do you have to buy clutch with off set in it ?

Depending on the vehicle and the way the arm is designed you may just have to do some minor fitting and filing.

I have a 1948 CJ2A. My clutch pedal arm is slightly bent to one side just above the 'elbow' or curved portion. Is this part supposed to be straight/vertical like the brake pedal arm?

The arm itself may be bent alittle. That is normal.
