do you connect the 12v hot lead to the center nut? and connect the horn button on the steering column to the other connections? I am having trouble connecting mine and getting it to work

I have a 1960 pickup. It has a horn relay and a steering wheel button but the horn is missing. I assumed that the ground connection was made when mounting the horn to the inner fender or firewall and therefore I'd need a horn with a single lead just for 12V. However, the horns shown here require two connections. Can you assist?

Yes, when you push the button the inner metal horn button cup makes the connection. Basically the horn is always hot.
