Will this fit a 1949 truck with the 4-134 L? It would've originally had a Carter WO but the truck isn't listed on the fitment for this model.

If you have a WO on yours then yes it is the correct carburetor. Some early Trucks did use the WO.

Is this an exact copy of the original Carter WO? I'm thinking of this as a direct swap-out spare replacement if/when needed but need to know if any tweaking or additional parts are needed.

Yes it is, the only thing it will not have is the vacuum port on the base of the carburetor base.
