Does the "UPPER BRAKE SHOE ADJUSTING ECCENTRIC" require both a lock washer and nut on the back side of the brake backing plate? Thanks.

Yes, you need the lock nut and lock washer on the backside.

The diagram for part # A-754 shows nut and lock washer under item #4. My order came without nut and washer. Or they included or separate, add part number it not included.
